   Medicare claims are handled electronically by medical providers. I have never seen a paper claim form for Medicare and doubt if it exists.

   All providers that treat Medicare beneficiaries are requried to file claims with Medicare. It does not matter if your provider accepts assignment or not. They must still file the claim on a timely basis.
